The Board of Examiners request that a new, standardized report form be used when reporting your annual moving expense reimbursements. Please use the format in the attached Excel file. It is in template form for easy use and contains a sample form showing how to fill in the fields. Note the entries are to be in chronological order of "Date of Initial Request" with unfilled positions labeled "vacant," " cancelled" or "delete-hired locally" noted in the employee name field. Please use this format for your 2003 moving reimbursement report.
Please continue to submit your reports by January 31 of each year. This should coincide with your IRS reporting for employees by calendar year .
Report only those reimbursements actually made to the employees in the reported calendar year. Any pending reimbursements at the close of the calendar year should be reported in the subsequent year's report.
